FDP-4 Equipment Information Sheet

This form is used to gather detailed information on inventoried equipment. When an asset has been received, this form must be completed in its entirety and submitted to Becky Key. The submission of this form will trigger the generation of a TAMU asset tag that will be placed on your asset. The assigned asset tag is the method used to track your asset for the life of the asset.  

FDP-422 Property Custodian Responsibility Statement

The purpose of this form is to document the responsibilities of the Property Custodian and for the Property Custodian to certify that they understand these responsibilities. This form is to be completed by the department head. 

FDP-423 Property End User Responsibility Statement

The purpose of this form is to document the responsibilities of the Property End User and for the Property End User to certify that they understand these responsibilities. This form is to be completed by the end user that is assigned the equipment. 

FDP-411A Property Transfers

This form is used to document the transfer of equipment from one department to another. It includes transfer between departments, out of or into TAMU. 

FDP-414 Property Additions and Deletions

The form is used to document an addition of equipment that did not require payment to acquire, and to document deletions of existing equipment due to sale, trade in, cannibalization, or damaged/destroyed/obsolete.

FDP_402 Certification of Physical Inventory Conducted by Department

Complete this form to certify the department's annual inventory per state requirements.

FDP-9 Report of Missing or Stolen

To report missing or stolen property or to recover a missing, disposed asset.
